So, what's the etiquette? You're walking down the street with your broadsword and a careless stranger [also carrying a broadsword] scuffs your new shoes. Is it just instantly "on" or does someone have to slap the other in the face with a glove?

You never slap, oh delicious one, the proper etiquette is to smash the other bugger's nose in with your mailed gauntlet.

But, yeah throwing it in their face is the right thing to do.

Throwing it in their face is un-chivalrous and rude.
Proper etiquette is to throw DOWN the gauntlet, i.e. near their feet. The challenge is accepted by picking up said gauntlet and handing it back to you. The challenge is refused by ignoring it.

I have been taught a very un-chivalrous method of duelling. When the other person bends down to pick up the gauntlet or glove, you draw your sword and run them through while they are on the ground. Naturally, that's not the way I conduct myself in combat.

The etiquette is to wait for the Président du Jury to say "Prêt, Allez." and then you cut their heart out.
